Root
Cause Analysis Training - Two-day

For
Operations Improvement, Manufacturing,
Reliability, Sentinel Event, Safety & Health, Quality, Maintenance,
Security
Root Cause
Analysis
Two-day REASON Training is available on your site, at our REASON Training
Center, and live on the Internet via WebCast. The training provides
personnel with the skills to break down an operations problem into its
essential parts, quickly model the system formed by those parts, and analyze
the model to identify root causes and best solutions. The methodology
identifies both the systemic causes and the appropriate prevention responses
to solve counter-quality operations problems. The training is a two-day,
structured course designed for individuals with on-the-job problem solving
responsibilities: manufacturing and operations quality professionals, safety
and reliability engineers, compliance and operations managers, and
production supervisors. The training equips attendees with the practical
skills to produce objective, concise, accurate, and consistent results
required in operations improvement and problem solving analyses.
Root Cause
Analysis
Training:
-
Provides a standardized, step-by-step,
objective method to identify and record the direct, contributing, and root
causes of occurrences and counter-quality conditions. Attendees learn a
logic-based, standard inquiry process that gets to the heart of breakdowns,
safety and health issues, scrap problems, missed schedules, wrong parts,
delays, errors . . . all of the things that negatively impact your
operation.
-
Provides an objective and structured methodology to model, validate,
record, and communicate the cause and effect relationships that are
producing problems in the operations of your organization. This is a
practical and no nonsense approach that works in the trenches on real
operations wastes.
-
Provides an objective method
that can be used to formulate and validate action plans. The class teaches
how you can demonstrate, before any resources are allocated, that your
“solution” will solve the problem.
-
You will learn how to measure
how much prevention benefit will be provided by each of your options. That
means that you can compare options to pinpoint your most effective option,
as well as determine which of your options is the most cost-effective.
Root Cause Analysis
Goals for Each Student:
-
Learn, understand, and apply the REASON Causal Patterns and methodology to
identify, model, and analyze root causes, contributing causes, and direct
causes of operations problems.
-
Learn, understand, and apply the REASON principles of organizational
control. Learn how to design into your operation’s programs, policies,
and procedures the sustaining controls that produce process improvement.
-
Learn
practical strategies and procedures for finding both the pervasive causes
that broadly impact operations day-to-day, as well to the critical core
causes of events requiring emergency response.
Root Cause Analysis Performance Goals
-
Learn
and apply the REASON logic technique in individual and group problem-solving
activity, as demonstrated by the ability to produce accurate causal factor
sets from classroom exercise materials.
-
Produce an accurate REASON Analytical Model graphically depicting the causal
process, including direct and root causes, as demonstrated by the ability to
produce with the REASON software an analytic model of an unwanted event that
will pass the application of logical necessary and sufficient testing.
-
Produce accurate and professional decision support documents and reports in
order to communicate the most efficient and cost-effective prevention
options and action plans, as demonstrated by the ability to generate and
explain a quantified REASON Event Model.
Root Cause Analysis Technology Transfer
REASON® training introduces conceptual and behavioral classroom goals that
produce solid-skills that are directly and immediately transferable to
professional analysis and reporting responsibilities. Problems are
identified and defined. An objective system for identifying direct
causes and root causes is provided. An objective method for
determining best control options is provided. An action planning method for
determining appropriate action steps is provided. An objective
methodology for verifying accuracy and completeness of data is provided. A
system of validating the process from investigation through action planning
is provided. A “proactive” method to audit policies for control
effectiveness is provided.
Root Cause Analysis Instruction Techniques
-
The
REASON Root Cause Analysis Training is a comprehensive learning experience:
-
Group
and individual analysis projects provide interactive sharing and
reinforcement.
-
Written classroom exercise worksheets practice and test participants on
concepts.
-
Computerized Gaming. Class divides into teams to compete in an entertaining
computerized board game that practices the participants in analytical
skills.
-
Role-Playing. Participants practice facilitating the class.
-
Hands-on Practice. The class uses REASON® Software to investigate and
analyze occurrences.
-
Interactive dialogue. Lecture and group discussion on key concepts,
concerns, and issues.
